Our new integrated suite of community services in the City aims to help all residents and workers access the services they need. We have a team of outreach workers who can work with people to identify what they need and to create a Community Support Plan which is a list of services and activities they have agreed they would like to use. We know from experience that some people will struggle to access services and this is where our Community Connectors come in. Community Connectors are fully trained volunteers who will work with an individual to help them access their support plan. This could mean accompanying someone to an appointment at the opticians or attending a social group with them for their first time or just ‘checking in’ over the phone regularly. Each case will be unique and dependent on what that individual needs and wants but the ultimate aim is to help them to build the confidence and motivation to be able to continue to access services independently once the Community Connector steps back.
